Forms to be returned School nurse (RHS, BF, or GW) by August 10th for the Fall/PreWinter season, December 10th for the Spring season. Athletes only participating in the Spring season can submit their forms in August and they will be valid for the entire year. If the health form is valid on August 24, 2024, it is valid for the entire 2024-25 year.

- One time Swim Test Certification Form (for Learn to Row)
- If taking the Swim test at the Wyckoff YMCA, this 2nd form is ALSO needed: Wyckoff YMCA Waiver

- Annual Ridgewood School District form Pre-Participation Physical form completed by a doctor.
- Health History Update form completed by the parent if the Pre-Participation Physical form was completed more than 90 days prior to August 24, 2024.
