• One time Swim Test Certification Form  (for Learn to Row)
  • ​If taking the Swim test at the Wyckoff YMCA, this 2nd form is ALSO needed: Wyckoff YMCA Waiver

Forms to be returned School nurse (RHS, BF, or GW) and [email protected] prior to season start​:

• Annual Ridgewood School District form Pre-Participation Physical form completed by a doctor. 
• Health History Update form completed by the parent if the Pre-Participation Physical form was completed more than 90 days prior to start date of season
